The aim of this study was to develop and reach consensus on a cariology teaching framework for dental schools in Spanish-speaking Latin American countries. A Delphi process—with a pre-defined agreement threshold of ≥80% among participants—was employed, consisting of three phases and guided by a Coordinating Group. During the Preparation phase, three panels of experts were selected and invited to participate: a) Associations Panel – regional academic/professional dental associations (n = 12); b) Dental Schools Panel – all existing dental schools (n = 263) across the 19 Spanish-speaking countries in the region; c) Peer Panel – international academic/professional association experts (n = 4). Drawing on consensus documents from Europe, Colombia, the Caribbean, the USA, Chile, and Spain—as well as updated scientific evidence—the Coordinating Group developed an initial framework proposal consisting of five domains, 10 main competencies (MC), and 92 specific competencies (SC). The Consultation-Agreement and Consensus phases involved three rounds of questionnaires. The MC were iteratively updated and shared with the Dental Schools Panel, while the SC were included for review by the Associations Panel. A variety of communication methods were used, including independent Google Forms questionnaires and interactive workshops. Final consensus was achieved after an in-person workshop with the Associations Panel, followed by a secret vote and an online meeting with the Peer Panel. A total of 127 academic/professional institutions participated: Associations Panel: 11 (91.6%); Dental Schools Panel: 112 (42.6%), representing all countries; Peer Panel: 4 (100%). The initial framework (5 domains, 10 MC, 92 SC) was revised based on feedback, resulting in a final consensus framework comprising 5 domains, 10 main competencies, and 85 specific competencies. A core cariology curriculum framework in Spanish for Latin American dental schools was successfully developed and validated through consensus among regional academic and professional institutions.
